Emergency Room 24 Hours (救命病棟24時) Returns October 12 — A Newcomer's Guide and What the Stars Say
Fuji TV’s medical drama Kyumei Byoto 24-ji (救命病棟24時, “Emergency Room 24 Hours”) comes back on October 12, 2026, its first new season in about 13 years. It is the sixth series, and the first to air in Fuji’s prestigious Monday 9 p.m. slot, with a 15-minute extended first episode. Yosuke Eguchi and Nanako Matsushima, the show’s two leads, reunite on screen for the first time in roughly 16 years.

What Is Kyumei Byoto 24-ji?
The series follows the emergency center of a large hospital. Its centerpiece is the relationship between Issei Shindo (Eguchi), a brilliant and demanding emergency physician, and Kaede Kojima (Matsushima), who first appears as a nervous new resident under his supervision. Each season picks a real social or medical problem in Japan and builds its stories around it. From 1999 on, the show ran in Fuji TV’s Tuesday 9 p.m. slot and regularly averaged close to 20% viewership.

Season 2 is the outlier: Matsushima was not in the main cast, and Eguchi carried it with Taiko Matsuyuki and Hideaki Ito. Otherwise the arc is clear. Kaede grows from trainee to director, while Shindo stays the cool, exacting mentor.
Where to start: Season 1 (1999) is the origin story and worth watching first. If you’d rather skip ahead, the new season starts with the two characters as veterans, so you can follow it without having seen the earlier ones.

In Their Own Words: The Eguchi and Matsushima Interview
In a newly released interview, the two leads talk about coming back. The quotes below are my translations from the Japanese.
On returning after so long. Eguchi says the long gap feels fresh rather than nostalgic: “I’m looking at it more with anticipation for what kind of drama it will be.” Matsushima says that each season has its own theme and she felt excited about “what they’d take up this time.” She adds that they had bumped into each other privately a few times, so it did not feel like meeting after 16 years.
A relationship that has changed. Matsushima explains that Kaede started as a trainee who bumped into Shindo, and that six series later she has grown alongside the character, who is now becoming head of the department. “Now we’re both veterans, so Kaede has more emotional room. I’m thinking of engaging Dr. Shindo in a more relaxed way.” Eguchi, who hears this for the first time in the interview, reacts with “You’re thinking that?” and admits they have not started filming. He notes that Shindo has always been cool and strict, so he is curious how relaxed the two can get after all these years.
Why the show still matters. Eguchi reflects that when the series began, an emergency-room drama felt special, about restoring life on the spot. Now there are many medical dramas. He says each country’s medical drama reflects its own healthcare system, but the core theme stays the same: how to face the patient and how to face life. That is why the show can still bring “emotion and a sense of hope.”
Modern medicine and today’s problems. Matsushima says that medical equipment has become so advanced over the last 10 to 20 years that even she and Eguchi were surprised, and she wants to show viewers what a real emergency room looks like today. Eguchi says the show will again take in issues such as workplace harassment and shift schedules, along with problems specific to Japan such as end-of-life care and disaster victims, with a wide range of patients in the stories.
Eguchi calls this “the new Kyumei of 2026,” made with a largely different cast and staff, and hopes it will “show a light of emotion and hope.”
Why It Matters
The Monday 9 p.m. slot is Fuji’s flagship “Getsu-9” drama block, and the network is betting on a familiar title in a crowded market. Some Japanese commentary has questioned Fuji leaning on past hits. Yet the show’s strength was never novelty. It is the pairing of Eguchi and Matsushima and an honest look at real medical problems in Japan. If the new season keeps to that, it could bring back longtime viewers and win over new ones.

かなり久しぶりなんですけど、逆にすごく新鮮に捉えていて。
どんなドラマになるのか、逆にそっちのほうが期待感があるという感じですかね。
松嶋菜々子
毎シリーズ、テーマがあるんですけれども、今回は何を取り上げるのかなという、ワクワクした気持ちでした。
江口洋介
こういう格好をして、また始まるんだなという感じはもちろんありますけれども、
もちろん、お互い全然違う役をいっぱいやってきているから、
また今やるとどんなふうになるのか、すごく楽しみですけどね。
松嶋菜々子
そうですね。役柄的にもちょっとポジションが変わって、成長していたりもするので、
そこで関係性も少し変わってくると思います。
それがどうなるか楽しみだなという感じです。
ちょっとプライベートで、たまたまばったり会ったりしたことが何回かあったんですよね。
なので、16年ぶりにお会いしたわけじゃないので。
江口洋介
そりゃそうでしょう。

そうですね。
病院ものといっても、救命という、ちょっと突出した、
その場で命を再生しなくちゃいけないという、
すごく判断力が必要な、特別なドラマを昔始めたんだなと、
スタートした時はすごく思ったんですけど。
それから、いろんなドラマがありますしね。
その国ごとに、医療ドラマは、その国が抱えている医療体制とか、
いろんなことも含めながら進んでいくんだけど、
結局は患者とどう向き合うか、命とどう向き合うか、みたいなものがテーマで。
だから、感動みたいなものとか、希望みたいなものをテーマとして
作品の中に入れ込めるという感じは、
すごく離れてみて、余計に思いましたね。
松嶋菜々子
そうですね。
楓は研修医から始まって、進藤さんにぶつかるところから始まって、
あれからもう6作目となると、それとともに自分も、
俳優としてのキャリアを役と少し重ねつつ、というところで。
役としては医局長になりつつ、というところで、
役とともに俳優としても成長してきた、
変化が見える期間だったなというふうに思います。
江口洋介
すごく時間もたっていますし、
どれだけ新鮮にというか、新しいものをやるような気分で。
スタッフもキャストも全然違いますし。
2026年版の新しい『救命』になるんじゃないかなと思って、
すごく楽しみです。
松嶋菜々子
役作りとしては、先ほども申しましたように、
研修医と指導医という立場から始まったんですけれども、
今は同じぐらいのベテラン、
先輩は先輩で変わらないんですけれども、
ベテランの域に達しているので。
そこの部分での気持ちの余裕とか、人間的な成長をした楓が、
進藤先生とどう向き合っていくのかというところで、
またちょっとラフさというか、
もう少し二人の関係的に、私がちょっとラフな感じで
進藤先生に絡んでいこうかなとか。
江口洋介
そう考えてる？ そう考えてる？

思っているところです。

なるほどね。
まだ全然撮影していないもんね。本当に。
そうか。なるほど。そうかそうか。
今までは進藤のほうがすごくクールで、
どちらかというとすごく厳しいタイプだったので、
その辺がまた時間がたって、
どのぐらいラフにやれるかは、逆に楽しみですね。
今聞いて、すごく楽しみです。

医療現場の器具も含めて、ここ10年、20年ですごく最新になって、
びっくりしましたよね。
江口洋介
びっくりしたね。

新しくなって、私たちも「えっ？」っていうものもあったりして。
そういうものも見せていきたいですし、
今の現場という、ERというものがどういうものなのかというのも、
今回ご紹介していきたいなというふうに思います。
江口洋介
医療というものが抱えている問題って、いろいろと、
パワハラの問題とか、シフトの問題とか、
その国々がいろいろ持っているものを
本当に入れ込んでいくみたいなんですけど。
今回もそういうものはあるんですけども、
でもやっぱり、それぞれの国が抱えている、
日本で言ったら延命処置の問題だとか、被災の問題とか、
幅広い患者さんがいっぱい物語の中に出てきますので。
それと医師たちがどう格闘、奮闘するか、
どう向き合うかという中で、
感動と希望の光を見せられるような作品になるんじゃないかなと思っていますので、
ぜひ期待してください。
